Navigating the aged care system alone can be challenging. With so many complicated options, knowing which one is right for you can be difficult. For most of us, the priority is to maintain independence for as long as possible. At Chic Disabilities, we believe you should be able to do this while receiving the quality care you need in the comfort of your home.

Make the most of your Support at Home Package

If you’re looking for tailored support to set up, manage, or transfer your Support at Home Package, you’re in the right place.


The Support at Home program is designed to assist you with daily activities such as cleaning, showering, and attending appointments.


All Support at Home recipients must work with an approved Support at Home provider to administer the Government subsidy.


Chic Disabilities is working with Trilogy Care, an approved Support at Home package provider, to coordinate your care and support. Chic Disabilities acts as your Care Coordinator, while Trilogy Care administers your Support at Home Package.


Trilogy Care’s model empowers you to have the flexibility to select your service providers and control how your Support at Home budget is spent. This will allow you to continue to work with your preferred care coordinator and service providers using your Support at Home Package budget. 


Working with Chic Disabilities and Trilogy Care means you will spend less on management fees, so you can spend more of your Support at Home budget on the things that matter—care and support services that will enable you to stay at home longer. 


The partnership between Chic Disabilities and Trilogy Care means you can receive Support at Home management support and services through Chic Disabilities. 

So, how does Trilogy Care support the care recipient?

Trilogy Care Supports our care recipients in managing their Support at Home with: 

  • Accounts: budgeting, planning, and payments 
  • Compliance support
  • Dedicated care coordinator
  • Individualised care planning and reviews
  • Phone access to nursing staff

Support at Home Inclusions and Exclusions

Services provided under a Support at Home Packages are based on an individual's assessed care needs, agreed care plan, and individual package budget.


Services that can be funded under the Support at Home Package program include (subject to your care plan and budget):

  • Bathing, hygiene, and grooming
  • Nursing
  • Allied health, such as Podiatry and Physiotherapy
  • Meal delivery and food preparation
  • Help with impairments and continence
  • Cleaning, laundry, and other chores
  • Home or garden maintenance
  • Changes to your home that are related to aging
  • Aids to stay independent
  • Transport
  • Social outings, groups and visitors
  • Care management


Services that can not be funded under the Support at Home Package program include: 

  • Household bills, including the purchase of food 
  • Insurances
  • Home appliances and white goods unrelated to ageing
  • Accommodation costs
  • Services that are funded under another government program, including:
    • Medication
    • Dental care
    • Ambulance services
    • Medical services, including gap payments

What is included in the Trilogy Care Self Managed PLUS fee?

Care Management

  • A dedicated care partner for every care recipient
  • The design and ongoing maintenance of care plans and care budgets including periodic care plan reviews.

Package Management

  • Management of the financial components of the Support at Home Package, including payment of invoices, reimbursements, monthly statements, and liaising with Services Australia.

Clinical Support

  • Phone access to Trilogy Care's in-house team of clinical nursing professionals to oversee the care recipient's care plan and any health concerns.

Compliance Support

  • Background checking of all service providers for relevant insurances and clearance
